HOSPICE PROGRAM at SOMERS MANOR

Somers Manor Nursing Home coordinates with Hospice & Palliative Care in managing hospice care.
Through a multidisciplinary approach, we care for our residents and provide support for their families.
Together, the team provides palliative care aimed at relieving symptoms and giving social, emotional and spiritual support.

The goals of hospice care are:

To affirm the cycle of life
To achieve an understanding of specific end-of-life issues from the perspective of each resident and
his or her family
To manage pain and symptoms in order to be more comfortable, enhance the resident's quality of life
and live with dignity
To attend to the psychosocial, spiritual and bereavement needs of the resident and their family
and friends.
