Admissions Information

Thank you for your interest in Somers Manor Nursing Home.

To start the admissions process, we will need the following documents:
  • A Patient Review Instrument (PRI)
  • Pre-admission medical SCREEN
These forms describe the care and services required by the applicant. For prospective residents who are applying from a hospital in New York State, the hospital staff will complete these forms. For applicants living at home, the forms are to be completed by an authorized home care agency, or a Geriatric Care Manager who is a certified PRI and SCREEN assessor.

Additionally, a completed Admissions Application Form must be submitted. Our Admissions Application (see below) may be printed, completed and mailed to:
